Dear Applicant

Are you a highly motivated individual and a team player with a passion and commitment to deliver successful innovative, quality services?  If you are, we are searching for outstanding individuals to join the team in Stoke on Trent Community Health Services.

Stoke on Trent Community Health Services will have an income of approximately £57M for 2009/10.  This is predicted to grow further during the year.  We have operated as an independent, autonomous organisation since July 2007.  Services are provided from 19 different health centres and clinics across Stoke on Trent as well as people's homes and in the community.  There are two community hospitals – Haywood Hospital and Longton Cottage Hospital.  Haywood Hospital is undergoing a major capital development.  The new £35M hospital will open its doors for local people in September 2009.  This development will further expand our services and includes a Stroke Rehabilitation Service.  We have 1500 people working with us to provide services to the local community.

There is an extensive and innovative range of community services for adults, children and young people that promote health and well being. The Rheumatology Service, based at Haywood Hospital, has a national reputation for its quality and range of services.  There has been significant investment in Intermediate Care Services and Community Matrons to keep people at home and to support early discharge from hospital.  Community Matrons are now using remote technology to monitor care in people’s home.  We also provide Urgent Care Services at the Walk-in Centre at Haywood Hospital and the Primary Care Urgent Care Unit.

We are continually improving our services for local people.  Stoke on Trent Community Health Services are the first community services in the country to be successful in attaining the Customer Service Excellence Award.  In our accreditation report we were commended for creating an open culture with a true focus on our customers.

We have been successful in attaining one of the national Transforming Community Health Services Leadership and Innovation Awards.  The aim of our Award is to improve even further how people access our services and receive the right care as quickly as possible.

Similarly, our commitment to our staff is shown by our two Leadership Programmes, Inspire and Innovate.  These programmes are attended by all people at Band 6 and above.  The Inspire Programme was acknowledged in the report of David Nicolson for 2007/08.  The performance in our staff survey for 2008/09 has improved and again shows a strong track of delivery.  We foster and encourage learning and sharing.  This is demonstrated by our Annual Celebrating Success event.

Best quality care is provided in partnership with local people and colleagues in local organisations.  Real time feedback is obtained from people who use our services.  We have strong partnerships with all our local NHS colleagues and our partners in Adult Social Care and Children’s Service.  We are strengthening our relationships with the Voluntary Sector and Practice Based Commissioning.

You will be a member of our corporate leadership team and have responsibility for ensuring success for the whole organisation as well as the specific function you will be leading. We work as a strong Leadership Team in both Stoke on Trent Community Health Services and with our partners to contribute towards improving health and well being.
